I apply to become an Independent Associate of Certified Travel Services, Inc.  I agree to pay to Certified Travel Services, Inc. a one-time fee of $995, a $50 per month service fee and royalties in accord with the Affiliation Agreement.  I understand there is a $8.00  fee for each air ticket that I book and that 100% of the commissions on travel that I book will be paid to me.

I certify that I possess no fewer than three years travel agency experience, hold an International Air and Travel Agent Network (IATAN) card or a Cruise Line International Association (CLIA) certificate, have generated not less than $100,000 in travel sales during the previous 12 months and have a 1099 Federal Tax Form for the last year showing at least $5,000 in earned commissions.

I submit herewith copies of my IATAN card or CLIA certificate and my 1099 Federal Tax form showing earned commissions for the last year.
